The risk of corneal infections associated with orthokeratology lens usage can hamper the growth of global orthokeratology market. However, prolonged overnight usage increases the risk of developing corneal infections. Several medical studies have found a higher occurrence of microbial keratitis, a serious corneal infection, in orthokeratology lens users as compared to normal soft contact lenses. For example, as per a  retrospective study from 2008-2014, across eight eye clinics in Hong Kong, orthokeratology lens usage accounted for about 44% of all contact lens related microbial keratitis cases, despite orthokeratology lenses constituting a small portion of the contact lens market. In  2021, according to a study analyzing 623 cases of contact lens related keratitis in the U.S. from 2015-2019, orthokeratology lens use was associated with the highest risk and worst visual outcomes compared to other types of contact lenses. Parents and individuals are wary of the infection risks, thus, decreasing its demand. Several government regulatory bodies like the U.S. FDA have issued warnings about orthokeratology lens usage, emphasizing the need for proper fitting and handling to reduce risks. Doctors are also reluctant to widely prescribe orthokeratology lenses until safety improves.
