Renal denervation is a minimally invasive procedure that aims to decrease the sympathetic nerve activity to the kidneys. It involves threading a catheter through the arteries until it reaches the renal arteries, which supplies blood to the kidneys. The device then delivers energy to the nerves in the vessel wall to disrupt the signals that can cause high blood pressure. The energy can be delivered through various methods, such as radiofrequency ablation, ultrasound, and more recently, direct injection of chemicals or new methodologies based on percutaneous delivery systems. The rationale behind renal denervation comes from the understanding that the kidneys play a significant role in blood pressure regulation. Overactive renal nerves can lead to increased systemic blood pressure. By disrupting these overactive nerve pathways, renal denervation can potentially help to sustainably lower blood pressure in people for whom other treatments have failed. Although the procedure showed promise in early clinical trials, some subsequent trials had mixed results, which led to further investigations to better understand the patient populations that may benefit and to optimize the technique.
