Endotracheal tube is a small, usually plastic tube inserted into the trachea through the mouth or nose to maintain an unobstructed passageway especially to deliver oxygen or anesthesia to the lungs. Types of endotracheal tubes include oral or nasal, cuffed or uncuffed, preformed (e.g. RAE (Ring, Adair, and Elwyn) tube), reinforced tubes, and double-lumen endobronchial tubes. For human use, tubes range in size from 2 to 10.5 mm in internal diameter (ID). It is done to keep the airway open to give oxygen, medicine, or anesthesia. Support breathing in certain illnesses, such as pneumonia, emphysema, heart failure, collapsed lungs, or severe trauma. The average size of the tube for an adult male is 8.0, and for an adult female is 7.0, though this is somewhat an institution-dependent practice. Pediatric tubes are sized using the equation: size = ((age/4) +4) for uncuffed endotracheal tubes (ETTs), with cuffed tubes being a one-half size smaller.
Cuffed endotracheal tube allows higher ventilation pressure in patients with poor pulmonary compliance, and it is used to monitor ventilation and tidal volume more accurately. The cuffed endotracheal tubes are designed to prevent the passage of fluid or air around the endotracheal tube. The uncuffed endotracheal tube has a larger internal diameter compared to a cuffed tube. It also has lower resistance to airflow. The uncuffed endotracheal tube allows easy suctioning and prevents injuries in the subglottic region. Endotracheal intubation can lead to complications such as tooth avulsion, oral-pharyngeal laceration, laceration or hematoma of the vocal cords, upper airway and nasal trauma, tracheal laceration, perforation, hypoxemia, intubation of the esophagus.
